If you are a patient registered at Farnsfield Surgery and have seen information about changes happening at the surgery, you do not need to take any action. Once a new provider has been identified in the New Year, we will let you know of changes at your surgery.
In the meantime, please continue to contact the surgery if you have a health concern or symptoms you are worried about. If you are a patient registered at Crown Medical Centre, you can continue to get services from your surgery as normal.
